Who we are
This website (fhcog.net) is operated by Family Health Care of Gallatin, PLLC, 831B Nashville Pike, Gallatin, TN 37066. This policy explains how the website handles information. It does not replace our HIPAA Notice of Privacy Practices, which covers your medical records (see below).
Please don't share health information on this website
Our contact form and chat assistant are not secure channels for medical information. Please do not include symptoms, conditions, medications, test results, dates of birth, insurance ID numbers or Social Security numbers. For anything about your health, call (615) 206-0500 or use the secure patient portal. In an emergency call 911; if you are in crisis call or text 988.
Contact form
When you send a message we collect your name, email, phone number and message. It is emailed to our office (through our email delivery provider, SendGrid), and you receive a confirmation email. The message is kept on our website server for up to 30 days and then deleted automatically. Google reCAPTCHA checks the form for spam when you use it; Google's Privacy Policy and Terms of Service apply.
Chat assistant
The chat assistant answers general questions about our clinic using an AI model (Google Gemini). What you type is sent to the AI provider to generate a reply. Chat conversations are not saved by our website. If you use "Message the office", your name, contact details and question are checked for health information, then emailed to our office and kept for up to 30 days. The assistant cannot give medical advice.
Online new patient paperwork and appointment requests
Our online paperwork and appointment request forms are Google Forms hosted in our practice's Google Workspace account, which is covered by a Business Associate Agreement with Google. Your answers go directly to our practice in Google and are not stored on this website.
Analytics and cookies
We use Google Analytics to understand how visitors use the site (pages viewed, approximate location, device type, and clicks such as "Call" or "Leave a review"). Our website host also uses PostHog analytics. Our chat box, forms and staff pages are excluded from session recording. These tools use cookies or similar browser storage. You can block cookies in your browser settings, and the site will still work.
Information stored on your device
If you finish our online paperwork and return to the site, your browser remembers that for up to 90 days so we can show a "Paperwork received" message. This is stored only on your device and you can clear it at any time by clearing your browser's site data.
Review page visits
When someone opens our review page or taps a "Leave a review" link, we count the visit or click by week. We don't collect names or other personal information for this.
Other services we link to
Our site links to or embeds services such as Google Maps, the healow/eClinicalWorks patient portal, healowPay, Google reviews, Facebook, Instagram and TikTok. Those services have their own privacy policies.
Your medical records: HIPAA Notice of Privacy Practices
How we use and protect your health information as a patient is described in our Notice of Privacy Practices. It is posted in our office, and a copy is available on request. Call (615) 206-0500 or ask at the front desk.
Children
This website is not directed to children under 13, and we do not knowingly collect personal information from children through the website. Parents and guardians can call the office for help with a child's care.
